Healthcare is a multi-trillion dollar industry in the United States, and yet many people are completely unaware of the brilliant people who work tirelessly behind the scenes to improve access, lower costs and improve patient outcomes. Welcome to PopHealth Podcast, the show by healthcare professionals that explores the challenges facing the American Healthcare system through the perspectives of the innovative minds looking to solve them and move the industry into the future. We invite thought-leaders and influencers from throughout the continuum of care to discuss population health; from elected officials tasked with setting public policy, to clinicians who treat patients in every conceivable condition and social situation, to ground-breaking business leaders who are creating ways to connect and streamline the delivery of care to an ever-growing and diverse nation. Our hosts, Gavin Ward & Zac Morgan, bring a combined 20 years of heath care and home care experience which allows them to facilitate thoughtful and meaningful discussion on topics including population health, care transitions, readmission prevention, etc. Our goal is to provide listeners with a wealth of knowledge on the issues which affect millions of Americans every day, and a glimpse inside the world’s largest healthcare system. The impact of loneliness and social isolation is an often-forgotten issue, and yet it is one that impacts our society more than most people understand.  When you consider that social isolation as you age can negatively impact your physical health in the same way that smoking a pack of cigarettes can, you begin to understand why.  Maureen Feldman runs the Social Isolation and Loneliness Collaborative at Motion Picture & Television Fund, an effort also backed by AARP and some of the leading aging organizations in Los Angeles.  She and I sit down to discuss this looming crisis and what they’re doing to fight it.

For many healthcare professionals, Dr. Mary Naylor and her transformative Transitional Care Model (fondly referred to as the “Naylor Model”), have become synonymous with care transitions and readmission prevention.  Adapted from a care transitions program for low birth weight infants, Dr. Naylor created one of the most influential models for caring for high-risk older adults.  With amazing results in hundreds of healthcare organizations across the country, it has become one of the standards for managing a high-risk patient population.  Now chair of the Care Culture and Decision-Making Collaborative through the National Academy of Medicine, Dr. Naylor continues to shape the future of healthcare through policy initiatives and research, including her recent 6-year term on MedPAC.

With the CBO Scoring of the Senate healthcare bill due this week, and a vote scheduled before the July 4th break, confusion and frustration regarding the future of healthcare in America and here in California is at an all-time high.  With this is mind, we present you with our timely and extremely topical interview with Secretary of the CA Department of Health & Human Services, Diana Dooley.  Secretary Dooley oversees a myriad of health services in CA, but two in particular seem to be under attack- Covered California and Medi-Cal.  However, unlike the doomsday predictions of many of the talking heads you’ll find on TV, Secretary Dooley (who describes herself as a ‘passionate moderate’) has remained calm in the face of extreme diversity and still believes that healthcare can be fixed.  Join us as Secretary Dooley walks us through where we are, and where we go from here.
