The Visiting Nurse Associations of America, better known as VNAA, has long been one of the pillars of advocacy for care in the home.  They’ve felt, however, that the industry overall remains too fragmented to work towards the common goals we all share.  This led them to create an entirely new entity, the recently unveiled elevatingHOME, through which they intend to “re-envision healthcare delivery to place home at the center of care.”  Gavin and Zac recently attended the kick-off ceremony during the Home Based Care conference in San Diego and spoke with the leadership of both organizations, and the thought leaders who are working to make this vision a reality.  Explore this enormous undertaking and visions of what the future can hold with guests including Tracey Moorhead, President & CEO of VNAA AND elevatingHOME, Bob Fazzi, Founding Board Member of elevatingHOME, and Erik Wexler, Chief Executive of Providence St. Joseph Health Southern California.

Brookdale is the largest senior living provider in the country, with more than 1,100 communities nationwide providing a range of Independent Living, Assisted Living, Memory Care and Skilled Nursing services.  Christian Lanham, VP of Business Development, sits down with Zac in beautiful Nashville, Tennessee to discuss what has made Brookdale so successful and why residential senior living is such an important option for individuals and their families throughout the aging process.  Additionally, as the healthcare industry continues to change and the aging landscape shifts so dramatically, we also discuss how Christian sees these communities as an essential, albeit often overlooked, pieces of the care continuum which can have huge impacts on short and long-term success of hospital and patient outcomes.
