Healthcare is a multi-trillion dollar industry in the United States, and yet many people are completely unaware of the brilliant people who work tirelessly behind the scenes to improve access, lower costs and improve patient outcomes. Welcome to PopHealth Podcast, the show by healthcare professionals that explores the challenges facing the American Healthcare system through the perspectives of the innovative minds looking to solve them and move the industry into the future. We invite thought-leaders and influencers from throughout the continuum of care to discuss population health; from elected officials tasked with setting public policy, to clinicians who treat patients in every conceivable condition and social situation, to ground-breaking business leaders who are creating ways to connect and streamline the delivery of care to an ever-growing and diverse nation. Our hosts, Gavin Ward & Zac Morgan, bring a combined 20 years of heath care and home care experience which allows them to facilitate thoughtful and meaningful discussion on topics including population health, care transitions, readmission prevention, etc. Our goal is to provide listeners with a wealth of knowledge on the issues which affect millions of Americans every day, and a glimpse inside the world’s largest healthcare system. When Dr. David Feinberg joined UCLA Health System many years ago as a psychiatrist, he wasn’t expecting to one day lead the organization.  However, after becoming CEO during a period of cratering patient satisfaction scores, Dr. Feinberg helped engineer one of the most incredible and well-documented cultural transformations in healthcare, establishing UCLA as one of the top 3-5 hospitals in the nation in the process.  While he’d quickly rebut that he was only a piece of the transformation, his ongoing routine of daily patient visits and emphasis on their satisfaction nevertheless inspired a book and a TEDTalk documenting UCLA’s success.  Now President and CEO of Geisinger Health in Pennsylvania, Dr. Feinberg oversees innovative programs like refunds for unhappy patients and genetic testing for entire populations, continuing an opportunistic vision that has served so many for good.

The Visiting Nurse Associations of America, better known as VNAA, has long been one of the pillars of advocacy for care in the home.  They’ve felt, however, that the industry overall remains too fragmented to work towards the common goals we all share.  This led them to create an entirely new entity, the recently unveiled elevatingHOME, through which they intend to “re-envision healthcare delivery to place home at the center of care.”  Gavin and Zac recently attended the kick-off ceremony during the Home Based Care conference in San Diego and spoke with the leadership of both organizations, and the thought leaders who are working to make this vision a reality.  Explore this enormous undertaking and visions of what the future can hold with guests including Tracey Moorhead, President & CEO of VNAA AND elevatingHOME, Bob Fazzi, Founding Board Member of elevatingHOME, and Erik Wexler, Chief Executive of Providence St. Joseph Health Southern California.
